Medication Refills

Please call us at least 24 hours in advance, preferably 48 hours in advance. Please leave us a message including:

• Child’s Name and Date of Birth
  
• Medication Name and Dose
• Allergies to any medications
• Pharmacy Name and Phone Number
  
• Your Name and Phone Number

ADHD Prescriptions must be picked up in the office and cannot be called into pharmacies.

Cancellations:
We understand that conflicts and emergencies sometimes arise. If you are unable to make an appointment, we appreciate at least 24 hours notice so that another family may be seen. Please call us at the office if you anticipate being more than 15 minutes late for your appointment.
